Our Support at Home Program (Categories 1–5) provides flexible, high-quality in-home aged care services to help older Australians live safely, confidently, and independently at home. Whether you need a little help with everyday tasks or specialised nursing and transition care, we’re here to make life easier, healthier, and more fulfilling.
We proudly provide culturally responsive aged care for people aged 65 and over, supporting older Australians from all walks of life to stay independent, socially connected, and healthy, in a way that honours their language, culture, and traditions.
With a compassionate team that speaks over 52 languages, we deliver multicultural aged care built on trust, respect, and cultural understanding, so you always feel supported in a way that reflects who you are.
How We Support You
We tailor every service to meet your needs through two key government-funded programs:
- Commonwealth Home Support Program (CHSP) – entry-level support for daily living and social connection.
- Support At Home (SAH) – coordinated and flexible care designed to meet your ongoing needs.
Who We Support
- Seniors aged 65 years and over who need additional help to continue living independently at home.
- People under 65 years living with a disability, dementia, or special care needs not met by other programs.
Our Services
We provide tailored aged care support at home across three key areas to help you stay safe, well, and connected:
1. Everyday Living
Keep your home clean, comfortable, and safe with:
- Domestic assistance
- Home maintenance and minor repairs
- Meal preparation and delivery
2. Independence
Stay active and socially connected with:
- Personal care and daily assistance
- Community and social activities
- Transport to appointments and social events
- Respite care for carers and families
- Therapeutic and wellness support for independent living
3. Clinical Care
Receive professional health and nursing support at home:
- In-home nursing care
- Nutrition and diet support
- Care management and coordination
- Allied health and restorative therapies
Short-Term & Specialised Support
When you need a little extra help, we also provide:
- Assistive technology and home modifications for safety and mobility
- Restorative care pathways to help you regain strength and confidence
- End-of-life care delivered with compassion and dignity
